I have read and I understand the "Policy Regarding the Legal and Ethical Uses of Technology Resources" for Our Lady of Sorrows Regional School ("the School").  I understand that disciplinary action could be imposed if I violate any of the policies and procedures regarding the use of the School's technology resources, including access to the Internet.

I understand that with the complex internetworking and access to systems worldwide through the Internet, the Network Administrator, faculty and staff can not control the content of information available through access to the Internet.  I understand that some systems that can be accessed via the Internet contain offensive material.  I understand that the School does not condone the use of such materials and will not permit access to, or the use of, such materials at the School.  I understand that violations of this policy could result in disciplinary action.

I understand that students will be allowed to access information resources via the Internet while at the School only under the direction and supervision of a teacher, and only if this signed permission form is on file in the School office.  Further, by signing this permission form, I understand that I hereby release the School and its employees from any and all claims of any nature arising from use of the Internet utilizing the School's technology resources.
