Welcome to the Registration Forms Page.

The forms are to be filled out ahead of time to save you time at registration.   Parents are required to attend school registration and you should bring your printed forms with you.  All forms must have original signatures and be returned to the school during the scheduled registration time period.

This is a great place to fill out forms if you have multiple children attending school.  

1.  To fill out a form click on a link below, once the form opens click in the first box and type directly online.

2.  Fill out the form for the first child and hit the print button at the bottom of the form (print 4 copies of the school registration form).

2. Go back to the form and edit the information for the next child.

3. Print 4 copies of the edited school registration form.
